Nevada HOSA Participation Forms

The Nevada HOSA Student/Advisor Participation Forms cover all in-state Nevada HOSA Sponsored Events for the school year.  The following forms are included:

To be completed once for all in-state events

  • Advisor Conduct (1 page): Submit online here
  • Student Permission/Medical Release Form (4 pages): Advisor keeps in their possession

To be completed and submitted before each in-state event

  • Advisor Statement of Assurance (1 page) Submit online here

HOSA Competitive Events and the Common Core State Standards

The Common Core State Standards are education standards that were developed by two national organizations in 2010 and adopted by 45 states. Many of today’s educators are looking to the standards as a guide toward improving student learning and achievement. Health Science teachers who serve as HOSA advisors tell us that the Common Core State Standards are an important part of their instructional focus.

Service Project



At the 2016 International Leadership Conference the Voting Delegates selected the National Alliance on Mental Illness (NAMI) as the 2016-2018 National Service Project.

According the the NAMI website, "NAMI provides advocacy, education, support and public awareness so that allindividuals and families affected by mental illness can build better lives. To meet that goal, NAMI is building a movement. We seek to broaden publicawareness and inclusion in every part of our alliance. We seek to increase ourvisibility and impact; strengthen our voice as a unified organization of livedexperiences and maximize our outreach to and engagement with all communities."
