The Nevada HOSA Student/Advisor Participation Forms cover all in-state Nevada HOSA Sponsored Events for the school year. The following forms are included:
To be completed once for all in-state events
- Advisor Conduct (1 page) Submit online below
- Student Permission/Medical Release Form (4 pages): Advisor keeps in their possession
To be completed and submitted before each in-state event
- Advisor Statement of Assurance (1 page) Submit online below

The Common Core State Standards are education standards that were developed by two national organizations in 2010 and adopted by 45 states. Many of today’s educators are looking to the standards as a guide toward improving student learning and achievement. Health Science teachers who serve as HOSA advisors tell us that the Common Core State Standards are an important part of their instructional focus.
At the 2022 HOSA International Leadership Conference Be The Match was selected as the 2022-2024 National Service Project.
According the the Be The Match website, “For the thousands of people diagnosed every year with life-threatening blood cancers like leukemia and lymphoma, a cure exists. Over the past 30 years Be The Match ® , operated by the National Marrow Donor Program ® (NMDP), has managed the largest and most diverse marrow registry in the world. We work every day to save lives through transplant.”
